Wound Care:  

 

At Big River Nursing & Rehab a holistic plan of care is designed for each our wound patients. Our staff is trained to and anticipate and care for their needs. A Nurse Coordinator, assigned to all wound patients, works closely with the family and the wound physician to monitor progress and serve as a resource to address concerns. The collaboration with the Primary Care Physician to ensure appropriate follow up care is coordinated upon discharge from Big River Nursing & Rehab. Our interdisciplinary team, Wound Physician, Licensed Dietician, Social Worker, Physical Therapist, Occupational Therapist, Speech Therapist and Nursing staff consistently reviews the individualized plan of care. This has reduced out average recovery time and stay by 2 to 8 weeks.

 

Memory Support:

 

Big River Nursing & Rehab is proud to offer a unique range of services for those residents with Dementia. Our staff is trained in the latest forms of therapy and care for the dementia patient, and we offer a warm, comfortable, and safe atmosphere. The daily routine and plan of care for the dementia patient will differ from individual to individual, but begins with a thorough analysis and consultation with the resident's caregivers, family, and medical team. We make sure to involve everyone with whom the resident is familiar and comfortable, so as to lessen the stress on him or her. Our daily activities on the Memory Support Unit are specially designed for the needs and interests of the individuals we care for. At Big River Nursing & Rehab, we focus on education and therapeutic programming in order to implement a "habilitation" model of care. Habilitation focuses on maintaining the skills, interests and abilities the resident has a proven successful model for treating the dementia resident. 

 

Dining Services:

We offer a homelike dining experience allowing the resident to make many choices daily. Residents often enjoy the socialization of a restaurant style atmosphere, while experiencing a home cooked meal. All meals are prepared in accordance with physicians orders and are directed by a registered dietitian, promoting the health and well-being of each individual.

Social Services:

Our Social Service Department participates fully in the development of a resident centered plan of care allowing an equal emphasis on psychosocial and health care needs. They also assist in admission and discharge planning.

Activities:

Our Activity Department is led by a licensed Activity Director who values resident opinion related to all planned activities. Big River Nursing and Rehab, LLC offers a multitude of stimulating activities, including outings to surrounding St. Louis attractions.
